Thus, neither front matter, such as title pages and acknowledgments, nor most appendixes, are factored into the final tally. Bibliographies, glossaries, and source citations are not indexed, while endnotes or footnotes are indexable when they “continue or amplify discussion in the text” (Chicago Manual of Style, 16.110).

My rates range from $4.50 to $6.50 per page, depending on the topic, complexity and density of the text, as well as the projected timeline. Pages are estimated to contain 350 words. Embedded indexes are charged at a higher rate.
